Our Adult Day Health Services program provides special care and socialization to people who are living at home, while also giving relief to their caregivers. Working with your community doctors, our program coordinates and manages each person’s health and medical needs. Our nurses monitor the status the frail elderly, chronically ill, and mentally and physically disabled adults in a day setting, allowing these individuals to remain at home and in their communities.
Open 7 days a week from 9 am to 3 pm, with door-thru-door handicapped accessible transportation provided.
Our program serves most parts of Suffolk and parts of Nassau County and is covered by Medicaid, long-term care insurance or private pay.
